Personal and subject matter jurisdiction are proper pursuant to the New York State Constitution and all other applicable laws, rules, and regulations.
On or about June 14, 2024, the Surrogate’s Court of Wayne County, New York issued Letters of Limited Administration empowering Plaintiff to prosecute this action.
At all relevant times, Mikac operated the aforementioned motor vehicle bearing Indiana license plate number 2996584 with the knowledge and consent of Bond or the true owner thereof.
As the result of the aforesaid injuries, Plaintiff’s Decedent sustained great physical and emotional pain and suffering, upset, distress, and fear of impending death.
As the result of Plaintiff’s Decedent’s wrongful death, his estate has incurred expenses and suffered pecuniary and other damages in an amount that exceeds the jurisdictional limits of all lower courts.

YOU ARE HEREBY SUMMONED to answer the verified complaint in this action and to serve a copy of your verified answer, or, if the complaint is not served with this summons, to serve a notice of appearance on the attorneys for the plaintiff within 20 days after the service of this summons, exclusive of the day of service (or within 30 days after service is complete if this summons is not personally delivered to you within the State of New York).
In case of your failure to appear or answer, judgment will be taken against you by default for the relief demanded in the complaint.
Erie County is designated as the place of trial pursuant to CPLR § 503(a).
