`I have conducted library research

`and services, including the location andretrieval of identified references, since

`1980.

`I specialize in documentretrieval and delivery of medical and
`
`biomedicalarticles obtained from the National Institutes of Health’s U.S. National
`
`Library of Medicine (“NLM”) in Bethesda, Maryland.
`

`Because I have no professional relationship with the NLM and simply
`
`access materials as a memberofthe public, I have personal knowledge about the
`
`library’s public access procedures. At the NLM, any memberof the public can
`
`enter the Main Reading Room on most weekdays (with the exception of federal
`
`holidays and other special or emergency closures) and can request materials from
`
`the NLM’s General Collection for pick-up at the Information desk.

`I have
`
`personally retrieved materials from the NLM in this mannerthousandsof times
`
`since 1980.

`In December 2017, I was asked by counsel for Griinenthal GmbH
`
`(“Griinenthal”) to acquire a copy ofan article titled, M. Muratoreetal., //
`

`Within a few days of receiving the request from Griinenthal, I
`
`personally went to the NLM to acquire a copy of Muratore. As was mygeneral
`
`practice in retrieving articles from the NLM,andconsistent with the library’s
`
`procedures for public access to materials in its General Collection, I entered the
`
`Main Reading Room, looked upthe call numberfor Progressi in Reumatologia in
`
`the library’s online catalog, and requested the specific issue containing Muratore.
`
`A library contractor or staff memberretrieved the hard copy of this issue, which I
`
`picked up from the Information desk.
`

`I then used the NLM’s public scanners to scan Muratore, along with
`
`the cover pages of the journal containing the article. The copy of Muratorethat I
`
`scannedis attached hereto as Exhibit A. At the bottom of each page of the scanned
`
`article is a marking which is added automatically by the NLM’s scanner,stating
`
`that “This material was copied at the NLM.”
`

`Ascan also be seen in Exhibit A, the cover page of the journal
`
`containing Muratorestates that it is the “Property of the National Library of
`
`Medicine,” and displaysa sticker stating “General Collection,” the journal’s call
`
`number, and a date/time stamp of “2004-07-19 10:33:34.” From mynearly forty
`
`years of experience retrieving articles from the NLM,I understandthatthis
`

`date/time stamp represents the date/time Muratore was received by the NLM, and
`
`that it would have been added to the NLM’s General Collection—and therefore
`
`available and accessible to the public through the procedures I have described
`
`above—within 7-10 days of receipt of the publication, depending on staff
`
`availability to process.

`I sent the scanned copy of Muratore and the journal’s cover pages
`
`(Exhibit A) via email to counsel for Griinenthal in December 2017.

`Il neridronatonel trattamentodell’algodistrofia simpaticariflessa dell’anca: confronto in aperto conil
`clodronato
`
`U.O. di Reumatologia Ospedale “Galateo” San Cesario di Lecce

`Lalgodistrofia simpaticariflessa rappresenta unasindromecaratte-
`rizzata da presenza di dolore localizzato, grave limitazione funzio-
`nale, turbe vasomotorie locali, osteoporosi secondaria localizzata,
`possibili alterazioni distrofiche a carico della cute. Il dolore, preva-
`lentemente di tipo meccanico, limita i movimenti impedendoil ca-
`rico. Radiologicamente sono, in genere, apprezzabili aree di demi-
`neralizzazione ossea irregolarmente estese (patchy osteoporosis),
`la
`scintigrafia trifasica documental’elevato turnover osseo. Gran parte
`degli studi, in letteratura, hannoriportato risultati positivi a seguito
`dell’utilizzo di farmaci antiriassorbitivi potenti, quali i bisfosfonati.
`Con l’avvento della RM l’algoneurodistrofia della testa del femore é
`divenuta pit facilmente documentabile, dato estremamente impor-
`tante in quanto se nontrattata adeguatamente puoevolvere verso la
`necrosi asettica dell’anca. Abbiamo volutoutilizzare in pazienti af-
`fetti da Algodistrofia riflessa il neridronato (Nerixia, Abiogen
`Pharma) un aminobisfosfonato, di recente commercializzazione,
`utilizzabile sia per via endovenosa che intramuscolare, gia speri-
`mentato nel trattamento di malattie ossee sia sistemiche (vari tipi di
`osteoporosi, osteogenesi imperfetta), che focali (morbo di Paget,
`ipercalcemie maligne), e valutare la sua velocitd di guarigione (di
`recupero,di restitutio ab integrum) della lesione, confrontandolo
`con un altro bisfosfonato in quantoil tempo di recupero funzionale
`di questa patologia é sostanzialmente molto lenta circa da 30 a 90
`gg con qualsiasi farmacosi utilizzi.
`Materiali e metodi. Abbiamo arruolato consecutivamente 18 pa-
`zienti (12 sesso femminile di eta media di 47 anni(39-57)e 6 uomi-
`ni di etd media 51 anni di eta media 50 (46-55), tutti affetti da Al-
`godistrofia della testa del femore (14 dx e 4 sx) con durata media
`della malattia di 4,1+ 2,0 mesi. La diagnosi é stata posta sulla base
`del quadroclinico, radiologico e risonanza magnetica nucleare. Per
`valutare lefficacia terapeutica del Neridronato i pazienti venivano
`randomizzate in due gruppi terapeutici: Gruppo A CLD 300
`mg/die (N=7) ev. diluiti in 250 ce di fisiologica per dodici giorni
`mentre al Gruppo B (N=11) é stato somministrato neridronato 100
`mg,e.v. diluiti in 250cc ogni 4 gg per 4 volte. I pazienti sono stati
`valutati alla visita basale (TO), dopo 15 gg (T1) e a 30 giorni (T2)
`mediante: scala analogicavisuale del dolore (VAS) e score clinico in
`cieco (scala arbitraria 0-3), markers di turnover osseo n-Jelopeptide
`(nTx normalizzati verso la creatinina), fosfatasi alcalina e osteocal-
`cina, prove di funzionalita renale ed epatica ( clearance creatinina,
`
`azotemia, transaminasi, GGT), valutazione sia in dinamica che in
`statica della pressione del carico d’appoggio podalico mediante esa-
`me podometrico con apparecchio “DINATTO 2,5”. In condizioni
`normali la valutazione sia in statica che in dinamica dell’appoggio
`podalico dimostra una equivalenza delle pressioni esercitate e della
`distribuzione dei carichi fra i due lati. In presenza di dolore di un
`piede Panalisi podometrica conferma un’alterazione dell’appoggio
`con unariduzione delle pressioni di carico dal lato del doloresia in
`statica che in dinamica. La radiografia e la RM venivanoeseguite a
`T0¢T2.
`Risultati. Al controllo T2, in entrambi i gruppi, l’'nTX, la fosfatasi
`alcalina e l’osteocalcina rilevavano la riduzione del turnover osseo,
`anche se gia a Ttale riduzione erasignificativa nel Gruppo B; a
`T2, Pimmagine radiografica documentava unariduzionedella rare-
`fazione ossea nei due gruppi, ma la RM_ evidenziava unarestitutio
`ab integrum completa della testa del femore nei soggetti in tratta-
`mento con Neridronato (Gruppo B) pit significativa rispetto al
`Gruppo A. Lavalutazione a T1(15 gg) ed a T2 (30 ge) dall’inizio
`del trattamento ha dimostrato, in particolare nel Gruppo B, unasi-
`gnificativa riduzione della VAS bencorrelata¢ la ridistribuzione del
`carico podalico come espressione “oggettiva® della riduzione della
`sintomatologia dolorosa. ma il dato piu sorprendente ¢,la velocita
`di recupero funzionale, quasi completo, gid dopo 15 gg nel gruppo
`con Neridronato Gruppo B.
`Conclusioni. Entrambi i farmaci si sono dimostrati efficaci nel
`trattamento dell’Algodistrofia Simpatica Riflessa ma, la velocita
`del miglioramento della sintomatologia dolorosa con recupero del-
`la capacita funzionale/motoria, la riduzione dei markers del riassor-
`bimentoosseo e la restitutio ab integrum della testa del femore si é
`dimostrata statisticamente pit significativa nei pazienti trattati con
`Neridronato. Sindrome simi! influenzale si sono manifestate in
`quattro pazienti del Gruppo B ben controllata dall’assunzione di
`paracetamolo, mentre funzionalita renale ed epatica non si sono
`modificate in nessunodei due gruppi.
