`CO., FLORIDA'S NOTICE OF PRODUCTION FROM NON-PARTY FOR 16
`SUBPOENAS DUCES TECUM TO NON-PARTIES FOR PLAINTIFF'S RECORDS
`SERVED TO PLAINTIFF ON JULY 10, 2023, and also regarding DEFENDANT, LANZO
`CONSTRUCTION CO., FLORIDA'S MOTION TO COMPEL PLAINTIFF TO SIGN
`AUTHORIZATIONS via zoom, on the 16th day of August, 2023, and the Court having reviewed
`the record and being otherwise fully advised in the premises, it is hereby,

`ORDERED AND ADJUDGED that:

`Plaintiff’s Motion is hereby GRANTED in part, as Defendant, LANZO CONSTRUCTION CO.,
`FLORIDA, is ordered to amend their 16 non-party subpoenas to limit their requests as follows:

`For medical records and bills requested, subpoenas must be limited to medical records and
`bills created 10 years prior to December 7, 2021, and only for medical records and bills
`related to treatment of Plaintiff’s left foot or left ankle.
`For pharmacy records and bills requested, subpoenas must be limited to pharmacy records
`and bills created 10 years prior to December 7, 2021, and only for medicines that are anti-
`inflammatory or pain reducers.

`For health insurance records and benefit payment requested, subpoenas must be limited to 
`health insurance records and benefit payment records created 10 years prior to December 7,
`2021, and only for records and benefit payments related to treatment of Plaintiff’s left foot or
`left ankle or for benefit payments made for medicines that are anti-inflammatory or pain
`reducers.

`Defendant, LANZO CONSTRUCTION CO., FLORIDA’s Motion is hereby GRANTED in part,
`as Plaintiff shall sign Defendant, LANZO CONSTRUCTION CO., FLORIDA’s authorizations,
`after the authorizations are limited by Defendant, LANZO CONSTRUCTION CO., FLORIDA
`as follows:

`For disability records and bills requested, authorizations must be limited to disability records
`and bills created 10 years prior to December 7, 2021, and only for disability benefit payments
`made to Plaintiff or requested by Plaintiff, related only to disability of Plaintiff’s left foot or
`left ankle.
`For Medicare records and bills requested, authorizations must be limited to Medicare records
`and bills created 10 years prior to December 7, 2021, and only for records related to
`Medicare benefit payments made regarding treatment of Plaintiff’s left foot or left ankle or
`for Medicare benefit payments made regarding medicines that are anti-inflammatory or pain
`reducers.
`For Medicaid records and bills requested, authorizations must be limited to Medicaid records
`and bills created 10 years prior to December 7, 2021, and only for records related to
`Medicaid benefit payments made regarding treatment of Plaintiff’s left foot or left ankle or
`for Medicaid benefit payments made regarding medicines that are anti-inflammatory or pain
`reducers.

`This order is made without prejudice to Defendant, who may renew their Motion at a later date
`upon further evidentiary findings which may warrant further discovery.
`
