FIG. 17 depicts a side elevation section View of the surgical stapling and severing instrument of FIG. 1 taken along the longitudinal centerline of the end effector in a partially closed but unclamped position gripping tissue.
FIG. 19 depicts a side elevation view in centerline section of the distal end of the surgical stapling and severing instrument of FIG. 1 in the closed or clamped position with tissue properly compressed.
The handle portion 20 includes a pistol grip 24 toward which a closure trigger 26 is pivotally drawn by the clinician to cause clamping, or closing, of the anvil 18 toward the elongate channel 16 of the end effector 12.
Returning to FIGS. 2—4, a distally prcscntcd cutting edge 48 between the upper and middle pins 38, 46 on the firing bar 14 traverses through a proximally presented, vertical slot 49 in the cartridge 37 to sever clamped tissue.

In order to use a circular sta- pler which has been adapted for performing a pyloro- plasty/pylorectomy, the anvil and staple cartridge, hav- ing a conventional pyloroplasty/pylorectomy shield mounted thereto, are positioned within a patient’s pylo- rus.
Conversely, if the shield member length is too long, then it may protrude a significant distance beyond the distal end of the anvil and can by its blunt, rough nature injure the back wall of the bowel as it is being intro- duced.
FIG. 8 is a perspective view of the pyloroplasty/— pylorectomy shield of the present invention inserted through a trocar into the stomach of a mammal and positioned in the pylorus prior to performing a pyloro- plasty/pylorectomy.
Typically, the patient is then insuf- flated with a sterile gas such as carbon dioxide in order to inflate the body cavity to give the surgeon room to maneuver the instruments.
5,445,644 shield 10 translates axially in a proximal direction caus- ing the anterior section 156 of pylorus 150 to be firmly engaged in the gap 12 between the anvil 35 and the distal end 62 of the stapler cartridge 60 effective to produce proper stapling.
