`Applicant has not been charged, arrested, or convicted of a criminal offense or offenses,

`except as provided below (omit minor traffic offenses):
`
`On October 23, 2012, the Pennsylvania State University Police Department charged me with
`purchase, consumption, possession or transportation of liquor by a minor. The incident took
`place at McElwain Hall, University Park, Pennsylvania 16802. As a result of the charge, I was
`sanctioned with a fine and summoned to the Commonwealth of Pennsylvania, County of Centre,
`Magisterial District Court 49-1-01. I entered into a provisional guilty plea before the Honorable
`Judge Carmine W. Prestia, Jr., and subsequently enrolled in the requisite Youthful Offenders
`Program ("YOP") for expungement. On December 16, 2013, after successfully completing YOP,
`my record was expunged of the above-identified incident. I have learned from my actions for
`which I take full responsibility.
