`I, the undersigned, hereby request that the Pretrial Release Agency staff investigate the alleged facts surrounding my arrest. charges, convictions or
`any other type of unlawful conduct, which may include the examination of both juvenile and adult records ofthe undersigned, in addition to
`conducting a general background investigation.

`In consideration of the Pretrial Release Agency's investigation of the possibility of my acceptance into, or, if accepted, my participation in the
`program, I the undersigned hereby release, remise and discharge the Galveston County Pretrial Release Agency, its board and employees,
`Galveston County and any and all law enforcement agencies, persons, firms, and/or corporations from any claims or liability whatsoever for the
`release of any information deemed necessary by the Pretrial Release Agency in the normal course of its business.
