`My name is AMANDA O’NEIL. I am currently Customer Service
`
`Manager, MN Sites at TE Connectivity (“TE”). I have personal knowledge of the
`
`facts set forth in this Declaration, except as otherwise stated. I am competent to
`
`testify as to all matters stated, and if called upon to do so, I would testify to the
`
`facts set forth in this Declaration.
`

`My compensation does not depend in any way on the outcome of this
`
`proceeding. I have not been offered any payment, incentive, or inducement to
`
`provide this Declaration.

`Medical Engineering & Design Inc., located at 2495 Xenium Lane N.
`
`in Plymouth, Minnesota (“MED”), was a company that specialized in making parts
`
`for the medical industry, and was in business in and around the year 2005. MED
`
`was later purchased by the Creganna Medical Group (“Creganna”). TE acquired
`
`Creganna in April 2016.
`

`The exhibits to this declaration were created in the ordinary course of
`
`MED’S business, when it was known by that name, and have been maintained in
`
`the ordinary course of business by MED, Creganna, and now TE.
`

`Exhibit 2089 contains complete, true and accurate copies of the
`
`following documents for an order placed by Vascular Solutions, Inc. with MED: an
`
`invoice (dated April 5, 2005), packing list (dated April 4, 2005), certificate of
`

`conformance (dated April 4, 2005), sales order acknowledgement (dated February
`
`28, 2005), quote and revised quote (dated February 11, 2005), purchase order

`Guideliner” (drawn February 10, 2005). These documents were retrieved from the
`
`files of TE, but originated with MED in 2005.

`Exhibit 2092 contains complete, true and accurate copies of the
`
`following documents for an order placed by Vascular Solutions, Inc. with MED: an
`
`invoice (dated June 16, 2005), packing list (dated June 16, 2005), certificate of
`
`conformance (dated June 16, 2005), sales order acknowledgement (dated April 12,
`
`2005), quote (dated April 11, 2005), purchase order (dated April 12, 2005), and
`
`engineering drawing for “Distal Section, Guideliner” (drawn/revised April 6,
`
`2005). These documents were retrieved from the files of TE, but originated with
`
`MED in 2005.
