`subsequently renamed ReShape Medical LLC. Effective as of October 23, 2017, EnteroMedics

`Inc. changed its name to ReShape Lifesciences Inc. ReShape Medical LLC has its principal
`
`place of business at 1001 Calle Amanecer, San Clemente, CA 92673 (the headquarters of the
`
`former ReShape Medical, Inc.) and is now a wholly-owned subsidiary of ReShape Lifesciences
`
