`gefunden werden kann. UnterstUtzung wurde hierbei
`durch verschiedene Unternehmen der Dentalindus(cid:173)
`trie geboten, die auf diesem Gebiet bereits Entwick(cid:173)
`lungsarbeit geleistet haben. So wurden beispielsweise
`die kompletten Garnituren konfektionierter Zahne
`von Candulor (CH-Wangen) und lvoclar Vivadent
`(FL-Schaan) digitalisiert und in das ZRS integriert. Die
`Suche im Zahnlager bleibt dem Zahntechniker somit
`erspart, weil das Programm ad hoc die zum Pati(cid:173)
`entengesicht passenden Zahne · anzeigt. Fur den fest(cid:173)
`sitzenden Zahnersatz und fUr Wax-Ups beziehungs(cid:173)
`weise die Oberpresstechnik hat das Unternehmen
`TEAMZIEREIS (D-Engelsbt-and) das Frontzahnasthetik(cid:173)
`System CALLAplus, bestehend aus 19 Oberkiefer- und
`drei Unterkiefer-Frnnten von den Zahnen 3 auf 3 als
`Modelle und Wachsfacetten, zur VerfUgung gestellt.

`Vorgehen
`
`Das Vorgehen fUr die Berechnung mit dem
`ZRS wird nun anhand des Beispiels einer Patientin
`dargestellt. Benotigt werden zwei digitale Frontal(cid:173)
`Aufnahmen der Gesichtspartie der Patientin: eines
`mit geschlossenem Mund und eines mit Jachelndem,
`leicht geoffnetem Mund. Beide Bilder werden in das
`Computerprogramm eingelesen (Abb. I).
`

`Jeder Zatmtechniker sieht sich bei der Ferti(cid:173)
`gung von Frontzahnversorgungen zunachst
`der Frage gegeni.ibergestellt, welche Zahn(cid:173)
`form fi.ir den spezif1schen Patienten passend
`ist. Fi.ir die Umsetzung von Totalprothesen
`steht eine Vielzahl von vorgefertigten Zahn(cid:173)
`garnituren im Zalmlager zur Verfi.igung. Fi.ir
`festsitzende Versorgungen, lmplantat- oder
`TeleskoparlJeiten wird in der Regel i.iber ein
`aufwendiges, zeitintensives Wax-Up ver(cid:173)
`sucht. die Frontsituation zu rekonstruieren.
`Dennoch fallen zahntechnische Frontzahn(cid:173)
`versorgungen hauflg auf den ersten Blick
`als ebensolche auf, da Zahnform und auch
`Zalmstel!ung sowie GrbP,enverhaltnisse
`nicht optimal mit dern Gesicht des Patienten
`harrnonieren und dadurch unnati.irlich wir(cid:173)
`ken. Um die Suche nach der passenden Form
`und Stellung zu vereinfachen, hat mein
`Vater ZTM Manfred Wiedmann ein spezielies
`Computersystem entwickelt.
`
`Durch die jahrelange Dokumentation und Analyse
`von Patientengesichtern und den zugehorigen Zahn(cid:173)
`situationen hat er GesetzmaBigkeiten hinsichtlich des
`Zusammenhangs zwischen bestimmten Gesichtsmerk(cid:173)
`malen und natUrlichen Zahnformen und -stellungen
`festgestellt. Aus diesen Erkenntnissen wurde eine pa(cid:173)
`tentierte Formel entwickelt, um computerunterstUtzt
`fUr jeden Gesichtstyp eine optimale, natUrlich erschei(cid:173)
`nende Frontzahnrekonstruktion zu berechnen. Mit der
`Anwendung dieses Zahnrekonstruktionssystems (ZRS)
`!asst sich die Fertigung von zum Patientengesicht dishar(cid:173)
`monisch wirkenden Restaurationen leicht vermeiden.

`unterschiedlichen Frontzahngarnituren hinterlegt wor·

`Das Bild mit geschlossenem Mund dient der Ge(cid:173)
`sichtsanalyse und wird hietiUr im Zeichenmodul des
`Programms geoffnet (Abb. 2). Auf der linken Seite
`der Bildschirmmaske befindet sich eine Tool-Leiste,
`mit deren Buttons die einzelnen Gesichtsmerkmale
`festgelegt werden. Zunachst wird der Umriss des Ge(cid:173)
`sichts mit dem Zeichenpad marl<iert. Dessen Form
`entspricht der umgekehn:en Form der mittleren ln(cid:173)
`zisive. Des Weiteren werden mit dem Grafiktablett
`der mittlere Stirnpunkt, die hochsten Punkte der
`Augenbrauen, die Position der Pupillen, die auf3eren
`Punkte der Nasenwurzel und der Nasenbasis in ihrer
`Breite, die Nasenspitze, der Winkel van Nasenspitze
`zu NasenflUgel sowie der Verlauf der beiden Mund-

`winkel (Lippenschlusslinie) und die Mitte des Kinns
`eingezeichnet (Abb. 3). Anhand dieser Angaben er(cid:173)
`rechnet ZRS die optimale Zahnform fUr die Patientin.
`Hiervon wird van der Software erst einmal eine ein(cid:173)
`fache 2D-Skizze erstellt (Abb. 4). Dann erfolgt auto(cid:173)
`matisch die Suche nach einem angemessenen Pendant
`innerhalb der im System hinterlegten Datenbank der
`verschiedenen Zahnformen. Nachdem die Suche er(cid:173)
`folgreich abgeschlossen wurde, wird das Ergebnis in
`einer dreidimensionalen Darstellung am Bildschirm
`angezeigt (Abb. 5 und 6). In dieser JD-Animation
`kann jeder Zahn einzeln mit der Mouse ausgewahlt
`und individuell durch den Anwender in der Stellung
`geande1·t werden. Die Nutzung dieser Option ist je(cid:173)
`doch normalerweise unnotig, da van dem Programm
`fU1· jeden Fall automatisch drei verschiedene Varian(cid:173)
`ten generiert werden konnen: eine gleichmaf3ige Auf(cid:173)
`stellung, eine leicht individualisie1·te und eine stark
`individualisierte.

`Im nachsten Schritt wird im ZRS auf das Bild
`der lachelnden Patientin gewechselt. Die berechnete
`Frontzahnaufstellung erscheint eingeblendet neben
`dem Fata. Zuerst wird Uber das Instrument ,,Pipette"
`die Farbe der Mundhohle bestimmt (Abb. 7) und die
`bestehende Versorgung ausgeschnitten (Abb. 8). Die

`Breite der neuen Versorgung wird gemaf3 der Breite
`der Nasenbasis festgelegt (Abb. 9). Die Position der
`Eckzahne befindet sich stets unter den auf3eren Punk(cid:173)
`ten der Nasenflligel. Als letzter Schritt wird die neu
`konstruierre Frontzahnaufsrellung in den leicht geoff(cid:173)
`neten Mund der Patientin eingeschoben (Abb. I 0).
`Die Zahnfarbe kann gemaf3 den Wlinschen der Pati(cid:173)
`entin im Rah men der VIT APAN classical Zahnfarben
`
`(VITA Zahnfabrik, D-Bad Sackingen) von Al bis D4
`verandert werden.
`
`Der Vorher-Nachher-Vergleich zeigt der Pati(cid:173)
`entin, wie ihre zukunftige Versorgung aussehen kann
`(Abb. I I). Es konnen die drei genannten Varianten
`zur Auswahl vorgestellt werden. Auf diese Weise er(cid:173)

`merkmale fUr die Festlegung der charakteristischen
`Punkte bei der Gesichtsanalyse. Daher muss eine
`Einfuhrung durch unser Fachpersonal in unserem
`Schulungszentrum in Steinheim e1iolgen. Nach einer
`solchen Einfuhrung kann das System aber umgehend
`im Labor- oder Praxisalltag angewendet werden, da
`es einfach zu bedienen ist. Die Teilnahme an einer
`Schulung ve1-pflichtet nicht zum Kauf des ZRS. lnfor(cid:173)
`mationen zu Schulungsterminen und Kosten sind auf
`der Unternehmenswebseite erhaltlich.

`Mit ZRS steht dem Zahntechniker und dem
`Zahnarzt ein hervorragendes Konstruktions- und
`Marketinginstrument zur Verfugung. In der Praxis
`kann das System fUr die tagliche Patientenberatung
`genutzt werden. Den Zahntechniker unterstutzt das
`Programm bei der Prothetikplanung, indem er - noch
`bevor er auf dem Mod ell zu arbeiten beginnt - weiB,
`wie das Endergebnis aussehen soil. Dies stellt einen
`wichtigen Baustein fur das Backward-Planning dar.
`Zeitintensive Umstellungen der Totalprothese oder
`Umschleifen und Umbrennen von Frontzahn1-ekon-
`struktionen aus Keramik konnen durch ZRS redu(cid:173)
`ziert werden. In unserem Labor konnen wir uns ein
`Arbeiten ohne ZRS nicht mehr vorstellen, da es fast
`taglich zum Einsatz kommt.

`In the production of anterior teeth restorations,
`each dental technician first considers the problem
`of which tooth shape is suitable for the specific
`patient. For the implementation of full prostheses,
`a large number of precast dental sets arc available
`in the tooth repository. For fixed restorations,
`implants or telescopic work, an expensive, time(cid:173)
`intcnsiH \\ax-up is usually attempted to
`reconstruct the anterior situation. However,
`dental technical anterior tooth rcstorntions often
`appear at first glance to be the same, since the
`tooth shape and the tooth position as \\ell as the
`size ratios do not harmonize optimal!~ with the
`patient's face and thus appear unnatural. In order
`to simplify the search for the right shape and
`position, my father Dental Technician Manfred
`Wiedmann developed a special computer S) stem.
`
`Support was provided by various companies in the
`dental
`industry
`that have
`already
`achieved
`developmental work in this field. For example, the
`complete sets of ready-made teeth from Candulor
`(CH-Wangen) and Ivoclar Vivadent (FL-Schaan)
`were digitized and integrated into the DRS. The
`dental technician is thus spared the search in the
`tooth repository because the program displays the
`teeth matching the patient's face ad hoc. The
`company TEAMZIEREIS
`(D-Engelsbrand) has
`provided the CALLAplus anterior teeth anesthetic
`system, consisting of 19 upper jaw and three lower
`jaw anteriors of the teeth 3 on 3 as models and wax
`facets, for the fixed dental prostheses and for wax(cid:173)
`ups or the press-on technique.
`
`Procedure
`
`Through years of documenting and analyzing
`patient faces and the associated tooth situations, he
`established
`the principles of
`the
`relationship
`between certain facial features and natural tooth
`shapes and positions. From
`these findings, a
`patented formula was developed to calculate an
`optimal,
`naturally
`appearing
`anterior
`tooth
`reconstruction for each
`type of face with
`the
`assistance of the computer. The production of
`disharmonious looking restorations can easily be
`avoided with
`the application of this Dental
`Reconstruction System (DRS) to the face of the
`patient.
`
`Selection
`
`In the DRS, a comprehensive selection of
`around 50 different sets of anterior teeth is stored so
`that the optimal solution can be found for every
`patient.
`
`The procedure for the calculation with the DRS
`is now shown with the example of a patient. Two
`digital anterior images of the patient's face are
`needed: one with the mouth closed and one with a
`smiling, slightly open mouth. Both images are
`scanned into the computer program (Fig. 1 ).

`The picture with a closed mouth is used for
`facial analysis and is opened in
`the program's
`drawing module (Fig. 2). On the left side of the
`screen is a toolbar, with the buttons with which the
`individual facial features are set. First, the outline of
`the face is marked with the drawing pad. Its shape
`con-esponds to
`the inverse shape of the mean
`incisor. In addition, the mid-forehead point, the
`highest points of the eyebrows, the position of the
`pupils, the outer points of the nasal base and the
`base of the nose in its width, the nasal tip, the
`comer of the nasal tip to the nostrils, and the course
`of the two comers of the mouth (lip closure line)
`and the middle of the chin (Fig. 3) are plotted with
`the drawing tablet.

`Based on this information, the DRS calculates the
`optimum tooth shape for the patient. A simple 2D
`sketch is first of al 1 created by the software (Fig. 4 ).
`Then
`the
`search
`for
`a
`suitable match
`is
`automatically can-ied out within the database of the
`various tooth shapes stored in the system. After the
`search has been successfully completed, the result is
`displayed on the screen as a three-dimensional
`representation (Figures 5 and 6).
`In
`this 3D
`animation, each tooth can be selected individually
`with the mouse and its position can be individually
`changed by the practitioner. However, the use of
`this option
`is normally unnecessary, since the
`program automatically generates three different
`variants for each case: an even lineup, a slightly
`individualized and a highly individualized one.

`In the next step, the image of the smiling patient
`is changed in the DRS. The calculated anterior
`dental set-up appears inserted next to the photo.
`First, the color of the oral cavity is determined by
`means of the "pipette" instrument (Fig. 7) and the
`existing restoration is cut out (Fig. 8).

`The width of the new restoration is determined
`according to the width of the nose base (Fig. 9). The
`position of the canines is always below the outer
`points of the nostrils. As a last step, the newly
`designed anterior dental set-up is inserted into the
`patient's slightly opened mouth (Fig. 10).
`
`The tooth color can be changed from Al to D4
`according to the wishes of the patient within the
`scope of VIT AP AN classical tooth colors (VITA
`Zahnfabrik, D-Bad Siickingen).
`
`The before-and-after comparison shows the patient
`how her future restoration might look (Fig. 11). The
`three variants mentioned can be presented for
`selection.

`Therefore, an introduction must be carried out by
`our expert staff at our training center in Steinheim.
`After such an introduction, the system can however
`be promptly applied in the laboratory or in practice
`every day because it is easy to use. Participation in
`a training course does not entail an obligation to

`In this way, the patient is not only given a notional
`treatment and cost plan, but is also given a clear
`picture of the aesthetic possibilities.
`
`Use
`
`With DRS, the dental technician and the dentist
`have an excellent design and marketing tool at their
`disposal. In practice, the system can be used for
`daily patient advice. The dentist supports
`the
`prosthetics planning program by knowing how the
`final result should look, even before he starts
`working on the model. This is an important building
`block for backward planning. Time-consuming
`changes of the full prosthesis or refacing and
`converting of anterior dental reconstructions out of
`porcelain can be reduced by the DRS. In our
`laboratory we can no
`longer imagine working
`without the DRS, because it is used almost daily.
`
`Prerequisite for useful results by the DRS is the
`correct
`interpretation of
`facial
`features
`for
`determining the characteristic points in the analysis
`of face.
