` direct the Cardiovascular Biomedical Research Unit, an NIHR funded translational research collaboration between Royal
`Brompton Hospital and Imperial College. The inaugural award allowed the design and build of a 10,000 ft2 research facility
`containing integrated 3T CMR and invasive cardiac angiography, echocardiography, biobanking and a next generation
`genetics laboratory. The second award incorporates translational research in 6 themes of: Advanced imaging, genetics &
`genomics, advanced heart failure, complex coronary artery disease, congenital heart disease, aorta & aortic valve.
`

` also direct the research in CMR, with 2 groups of researchers: a) clinical research staff of 4 CMR consultants, 5 cardiology
`fellows, 5 international cardiology fellows, and 2 research technologists; and b) technical development staff with 5
`physicists, 2 engineers, and 2 computer scientists which allows new sequence design, scanner optimisation and
`development of post-processing techniques. I also direct the core laboratory in CMR which analyzes data from
`international multicentre CMR trials.
`
`The current clinical research themes using CMR are:

`Use of myocardial T2* to identify and treat myocardial siderosis in thalassaemia major
`Phenotyping of cardiomyopathy by CMR with gene discovery
`Diffusion tensor imaging of myocardial architecture, especially for cardiomyopathy
`Assessment of prognosis in cardiomyopathy
`Myocardial perfusion imaging
`Pre-symptomatic assessment of atherosclerosis
`
`RESEARCH OUTCOMES
`
`The discovery and implementation worldwide of T2* CMR to measure cardiac iron has almost eliminated cardiac
`related deaths from heart failure due to iron toxicity in countries with widespread reach into the thalassaemia
`community. Documented decreases in mortality from heart disease was 71% in the UK after introduction of T2*
`before the thalassaemia registry was closed to the low number of residual events. It is estimated that there is
`potential to save 30,000 lives per year worldwide with the use of this technology. T2* CMR is now in all
`international guidelines for management of thalassaemia.
`
`We have described the use of late gadolinium enhancement CMR in numerous cardiomyopathies, to describe
`unique diagnostic patterns of fibrosis that are now routinely used worldwide. In addition, we have gone on to
`prove that both the presence and the burden of fibrosis predict future cardiovascular events that indicate the
`need for implanting a cardioverter-defibrillator to abort sudden cardiac death. In this regard late gadolinium CMR
`appears to be superior to ejection fraction alone in predicting outcomes. This practice is now in international
`guidelines.
`
`We have a world leadership position in the clinical application of diffusion tensor imaging of the heart and have
`shown substantial deficiencies in myocardial sheetlet function in cardiomyopathies that are diagnosis specific. We
`are currently applying DTI in a range of pathologies in the integration of this disruptive technology into clinical
`practice.
`
