`1. I am an experienced litigation attorney with more than twenty years of

`experience.

`2. I am a member in good standing of the Bar of the State of Illinois. I am also
`
`admitted to practice before the following courts: United States Supreme Court,
`
`Federal Circuit Court of Appeals, Court of Appeals for the Fifth Circuit, Northern
`
`District of Illinois.
`
`3. I have not been suspended or disbarred from practice before any court or
`
`administrative body. I have not had an application for admission to practice
`
`before any court or administrative body ever denied.
`
`4. I have read and will comply with Office Patent Trial Practice Guide and the
`
`Board’s Rules for Practice for Trials, as set forth in Part 42 of 37 C.F.R.
`
`5. I agree to be subject to the United States Patent and Trademark Office Rules
`
`of Professional Conduct as set forth in 37 C.F.R. §§ 11.101 et seq., and
