`3. On March 3, 2017, the Alkermes facility in Wilmington, OH received a

`package from Ashland shipped via DHL Express (Tracking Number:

`5132059236). The package was approximately six inches in length, six inches in

`width and 12 inches in height.

`I did not open the package.
`

`I provided the package to a courier service on March 6, 2017 called Quick
`
`Courier with the request that they deliver it to the following address: Professor
`
`Stevin Gehrke, 1036 Hartland Drive, Lawrence, KS 66049.
`
`I signed a chain of
`
`custody form supplied by Quick Courier.
`
`I declare that all statements made herein on my own knowledge are true and that
`
`all statements made on information and belief are believed to be true, and further,
`
`that these statements were made with the knowledge that willful false statements
`
`and the like so made are punishable by fine or imprisonment, or both under Section
`
`1001 of Title 18 of the United States Code.
