`I, Jennifer Seraphine, declare as follows:

`I have been practicing in the field of intellectual property, and

`in particular patent litigation, for over seventeen years.

`I am a member in good standing of the Bars of the State of

`California, State of New York, District of Columbia, and am an inactive
`
`member of the Florida Bar. I am also admitted to practice before the
`

`In the past three (3) years, I have appeared pro hac vice in one

`proceeding before the United States Patent and Trademark Office, Ivera
`
`Medical Corporation v. Catheter Connections, Inc., IPR2014-01124.
`

`I am familiar with the subject matter at issue in this proceeding.
`
`I have reviewed the ’678 patent that is subject to Inter Partes Review in this
`
`proceeding and the prior art references that are asserted by petitioners to
`
`invalidate that patent. I have also assisted lead counsel Mr. Smith in the
`
`drafting of the IPR petition for this patent.
