`Court for the District of New Jersey, Civil Action No. 95-1135 ("Tyco") as a result of

`seeing the report case at

`We subsequently contacted the Clerk of Court of the District of New
`
`Jersey to request the Tyco case file be made available at the Mitchell H. Cohen
`
`Building & US. Courthouse at 4th & Cooper Streets, Camden, NJ 08101 for my
`
`review.

`On Tuesday, May 6, 2014, I personally went to the Mitchell H. Cohen
`
`Building and US. Courthouse at 4th & Cooper Streets, in Camden, NJ 08101 to
`
`review the case Tyco file.
`

`On Monday May 12, 2014, I returned to the courthouse with personnel
`
`from CopySecure USA Pty Ltd, a litigation support firm, for the purpose of scanning
`
`selected Tyco documents and accompanying exhibits associated with Tyco docket
`
`numbers: 1, 7, 9, 13, 14, 15, 23, 26, 28, 29, 30, 31, 50, 52, and 57 in color.
`

`On Wednesday, May 14, 2014,
`
`I personally printed the scanned
`
`documents and accompanying exhibits and presented them to the Clerk of Court at
`

`the Mitchell H. Cohen Building and US. Courthouse for certification as true and
`
`correct copies of the selected docket entries.

`On Thursday, May 15, 2014, a courier returned the documents to our
`
`offices at 30 S. 17th Street, United Plaza, Philadelphia, PA 19103.
`

`Upon receipt of the certified documents, they were scanned into our
`
`electronic records.

`I personally supervised the individual making the copies of the Tyco
`
`documents being submitted in this matter.
`
