`Six Healthtare Organizations
`Select Health Hero Network, Inc,
`for Disease Management Programs
`
`ealth Hero Network, Inc. (HHN), a
`privately held e—health company fo-
`cused on developing innovative so—
`lutions to improve the exchange [0 infor—
`mation between care providers and pa-
`tients, announced that it has signed agree-
`ments with 6 organizations across the
`United States to implement a wide variety
`of disease management programs based on
`teh Health Hero Network Online Service,
`which links healthcare providers to pa—
`tients with the Health Buddy, andd easy—
`to-use information appliance that sits at
`the point of self—care in the home. The or-
`ganizations are conducting programs on
`early symptom management for coronary
`artery bypass graft (CABG) surgery, con-
`gestive heart failure (CHF), diabetes, asth-
`ma, mental health, and senior well being.
`The sites include 3 academic centers
`using the service in controlled studies:
`Stanford University School of Medicine,
`Department of Psychiatry and Behavioral
`Sciences (depression, bipolar disorder and
`obsessive-compulsive disorder); the Uni—
`versity of Nebraska Medical Center (CHF
`and CABG); and the Children’s Hospital
`Oakland, Ambulatory Pediatric Clinic, in
`conjunction with the School of Public
`Health at the University of California
`Berkeley (juvenile asthma).
`The other organizations are piloting
`new or existing disease management pro-
`grams based on the Health Hero Network
`Online Service. They include: Telesis Med-
`ical Management, Inc, a White Plains,
`New York-based medical management
`company that works with individual prac-
`tice associations (CHF and asthma); Santa
`Clara County Individual Practice Associa—
`tion Medical Group, in San Mateo, Cali—
`
`fornia (CHF and diabetes); and Bridgeport
`Hospital, in Bridgeport, Connecticut, and
`its affiliate home care agency, United
`Home Care (senior well being and CHF).
`The University of Nebraska Medical
`Center (UNMC) is the first academic cen-
`ter to implement a disease management
`program with HHN. In its controlled
`study, healthcare providers at UNMC‘s
`School of Nursing utilize the Health Hero
`Network Online Service to transmit im—
`portant and timely information to 2 sets of
`patients at home: CHF patients and post—
`operative CABG patients.
`“With earlier discharges from the hos-
`pital, we need an efficient way to closely
`monitor large numbers of patients at risk
`for symptom management problems and
`complications, to enhance their recovery
`in a timely manner,” said Lani Zimmer-
`man, associate professor of nursing.
`“Health Hero Network offers a significant
`advantage in managing these patient popu—
`lations, and we believe it will help us iden-
`tify serious situations with our patients
`sooner, with the goal of leading to less in-
`vasive and less costly interventions.”
